Some interesting blips in the phrase's timeline include boxer Norman "Kid McCoy" Shelby and Bill McCoy's pure alcohol during prohibition. Norman Shelby was so envied that many up-and-commers started to share his namesake to the point at which he had to specify that he was the real McCoy. Bill McCoy gained his recognition during the American prohibition. When there was mostly watered-down alcoholic beverages, McCoy took a boat to Canada for real booze and sold it in the international waters off of New York City.
The Scottish "Reay Mackay" [source] is a supposed relative of the phrase from the 1880s. The real Macao is a name for pure heroin from East Asia [source] as well as a play on the popular phrase.
